In anime and manga (and I guess real life), how do the Japanese school sections work?

Question by Rerednaw: In anime and manga (and I guess real life), how do the Japanese school sections work?
While I watch and read a bit...I've never figured out the Japanese school system. What is 2B versus 3F? Do they go by year? Are the letters denoting general scholastic ability? Is A better than C?

What's the equivalency to the American system? (9-12 grade = ??? in Japan)
